Ethics Complaints Procedure

Procedure for Receiving Complaints

Complaints alleging violation of the Town’s Code of Ethics Ordinance shall be submitted as follows:

  1. All complaints must be submitted in writing using the Town of Middlebury Ethics Violation Complaint Form.  Forms may be obtained from the Town Clerk’s Office located within Town Hall or online at http://www.middlebury-ct.org.
  2. All Ethics Violation Complaint Forms must be signed, under penalty of false statement (C.G.S. 53a-157), before a Notary Public.
  3. Ethics Violation Complaint Forms may be submitted to the Ethics Commission by mail or by hand-delivery in a sealed envelope marked “Confidential” to the following address:
