Certified Copies of Vital Records:

Certified copies of Vital Records are available in person or through the mail (Middlebury Vital Statistics, 1212 Whittemore Road, Middlebury CT 06762). All mail requests are processed the day they are received. Please refer to the appropriate application form and identification requirements below.

Vital Statistics Guidelines:

Due to a recent increase in identity theft, the Office of the Town Clerk would like to remind citizens of the guidelines for obtaining a copy of a birth, death or marriage certificate. These guidelines, developed by the State of Connecticut, are crucial in maintaining the rights and safety for all individuals.


Accessing Birth Certificates:

Access to birth records less than 100 years old is restricted in Connecticut. Pursuant to Connecticut General Statues, the following are the only persons who may have access to or be permitted to examine the original or any copy of a birth certificate. Telephone verification of any information concerning a birth record is prohibited.

  • The person whose birth is recorded, if over eighteen years of age (sixteen for a short form only);
  • Such person’s children, grandchildren, spouse, parent, guardian or grandparent;
  •  Attorneys-at-law representing such person or such person’s parent, guardian, child or surviving spouse;
  • A conservator of the person appointed for such person;
  • Members of genealogical societies incorporated or authorized by the Secretary of State;
  • Agents of a state or federal agency as approved by the State Department of Public Health.

Any eligible party requesting a copy of a birth certificate must provide verification of the relationship entitling them to a copy. Additionally, the requester must submit a government issued photographic identification (valid driver’s license). If a Government issued photo ID is not available, at least two of the following documents are required:

  • Social Security Card;
  • Social Security card supplemented with either an employment ID, a paycheck stub or a W-2 form;
  • Automobile registration;
  • Copy of utility bill showing name and address;
  • Checking account deposit slip or bank statement showing name and current address;
  • Voter registration card;
  • Valid government issued trade or professional license;
  • Valid government issued firearm permit;
  • Probation documents issued by a court or other government agency, pursuant to a criminal conviction;
  • Letter (dated within six months) from a government agency verifying identity;
  • Release documentation from a correctional institution containing a photograph of the former inmate and a release date (within 12 months);
  • Military discharge papers;
  • Current school or college photographic ID.

Accessing Marriage or Death Certificates:

Any person may obtain a copy of a marriage or death certificate (subject to Freedom of Information Act). Social Security Numbers will be removed as applicable.

Vital Statistics Fees:

Vital Statistics Fees – (cash or check made payable to TOWN OF MIDDLEBURY):

  • Birth Certificate (long): $20.00
  • Birth Certificate (short): $15.00
  • Marriage Certificate: $20.00
  • Death Certificate: $20.00

Application Forms for certified copies of birth, marriage or death certificates must be completed and submitted along with the necessary ID requirements and payment.


Marriage License:

If you are getting married, you must obtain a marriage license in the Connecticut Town where the marriage is to take place. A Marriage License Worksheet must be completed in its entirety.  The Marriage License Worksheet is not the actual Marriage License. Complete and email the worksheet and copies of Government issued photo ID's to townclerk@middlebury-ct.org.  In the body of your email please provide the date of marriage and when you would like to schedule your appointment.  A staff member will contact you to arrange an appointment.  Appointments are required for all Marriage License requests and may be made between Monday and Friday, from 8:15 a.m. - 3:30 p.m.

When completing the worksheet:

  • For Spouse 1 / Spouse 2 name - please include full middle name (if applicable);
  • "Mothers Maiden" name (birth name) includes full first, middle (if applicable) and last name;
  • "No. of this Marriage" - if this is your first marriage you will put a 1 in that box.  If this is your second marriage put a 2 in that box etc.
  • You may omit the section requesting social security numbers.  This can be provided at the time of appointment.

Keep in mind:

  • Both parties to a marriage must be at least 18 years of age to apply for a license; 
  • Both parties must appear in person with your original marriage license worksheet and photo id's;
  • A Marriage License is valid for 65 days from the date it is initially issued;
  • There is a $50.00 statutory fee (cash, check or credit card) to obtain a Marriage License.  There is an additional $20.00 fee for each certified copy requested.  Certified copies are needed for changing the Last Name, Insurance, ID/Driver's License, Social Security, Passport etc.
