General Information
The Town Clerk’s office is entrusted with the responsibility of recording and indexing all of the town's land records; issuing and recording marriage licenses and recording birth and death certificates; issuing dog licenses; recording military discharges; filing liquor permits; keeping notary, justice of the peace and campaign finance records; performing a variety of election related duties, including the issuance of absentee ballots; filing and posting of agendas, minutes and supplying general information to the public
Absentee Ballots
Electors are eligible to vote by absentee ballot for the following reasons:
- Active service in the Armed Forces
- Absence from town during all of the hours of voting
- Illness or physical disability
- Religious tenets forbidding secular activity on the day of voting
- Duties as an election official at a different polling place
Absentee Ballots may be obtained from the Middlebury Town Clerk’s Office either by mail or in person. An application must be filled out and received before an absentee ballot can be issued. Applications for Absentee Ballots and Voter Registration Forms are available at the Town Clerk’s office or can be downloaded from the Secretary of the State’s website at www.sots.ct.gov under Elections and Voting.

Dog Licensing Information
Dogs are licensed each year during the month of June and licenses can be obtained via either walk-in or mail-in registration.
All new and renewal licenses require a copy of your dog's current rabies certificate from a veterinarian. Licenses for a new dog that has not been licensed in the prior year also require a copy of the spay/neutered certificate, if applicable.

If licensing via walk-in registration, be sure to bring the required documents and payment (cash or check) with you at the time of application. If licensing via mail-in registration, a self-addressed stamped envelope would be appreciated in addition to the required documents and proper payment.

Notary Public Information
The Town Clerk and Assistant Town Clerk are Notaries. The fee to notarize a document is $5.00 per seal. Photo I.D. is required. Wills, Living Wills and Trust Agreements are excluded from this service.
Veterans Discharge
All Veterans who served during War Time are eligible to a reduction on the assessed value of their property, if they own property in town, or on their motor vehicle if they do not own property in town, by recording their DD-214 (Discharge Papers) in the Town Clerk's Office. These are then forwarded to the Assessor's Office to be applied to your taxes. The cut-off date is October 1st in order to affect your following July's tax bill.
Duplicate copies of DD-214's can be obtained by requesting and completing a Standard Form (SF-180) Request Pertaining to Military Records, to the National Personnel Records Center, Military Personnel Records, 9700 Page Boulevard, St. Louis, MO 63132-5100. Trade Name Information
State of Connecticut General Statute Section 35-1(a) requires any person doing business in Connecticut, except as provided by statutes, under any name other than the name of the person or persons conducting or transacting business, must register a Trade Name Certificate with the Town Clerk's office in the town where the business is or is to be conducted or transacted. The filing fee for a Trade Name Certificate is $5.00. Trade Name Certificate Forms are available in the Town Clerk's office.
Vital Statistics
Certified copies of vitals are available in person or through the mail (Middlebuury Vital Statistics, 1212 Whittemore Rd., Middlebury, CT 06762). All mail requests are processed on the day they are received in our office. If you choose to come in person we will fill requests for birth, death & marriage certificates at that time.
Accessing Birth Certificates:
Access to birth records less than 100 years old is restricted in Connecticut. Pursuant to Connecticut General Statutes, the following are the only persons who may have access to or be permittted to examine the original or any copy of a birth certificate. Telephone verification of any information concerning birth or birth record is prohibited.
- The person whose birth is recorded, if over eighteen years of age (sixteen for a short form only);
- Such person's children, grandchildren, spouse, parent guardian or grandparent;
- Attorneys-at-law representing such person or such person's parent, guardian, child or surviving spouse;
- Members of genealogical societies incorporated or authorized by the Secretary of the State to do business or conduct
affairs in this state;
- Agents of a state or federal agency as approved by the State Department of Heath.
Any eligible party requesting a copy of a birth certificate must provide verification of the relationship entitling them to a copy of the birth certificate. Additionally, the requester must submit the following documentation (Only these documents are allowed as proof of identification). If a Government issued photographic identification or if a photo ID is not available, at least two (2) of the following documents are required:
- Social Security Card;
- Written verification of identity from employer;
- Automobile registration;
- Copy of utility bill showing name and address;
- Checking account deposit slip showing name and address
- Voter registration card.
The charge is $20.00 for a certified copy of the full birth certificate and $15.00 for a certified copy of a short form birth certificate. Payment can be made with cash or a check made payable to the Middlebury Town Clerk.
Accessing Marriage, Civil Union or Death Certificates:
Any person may obtain a copy of a marriage, civil union or death certificate (all documents subject to the Freedom of Information Act). Social Security Numbers will be removed as appplicable, in accordance with Public Act 97-7. The charge is $20.00 for a marriage, civil union or death certificate, and all copies will be certified with a raised seal.
Application Forms for certified copies of birth, marriage, civil union or death must be completed and submitted along with necessary ID requirements and payment.
